3K a year Job Purpose Licensed Professional is the Intensive In-Home Services (IIH) Team Lead who is responsible for providing IIH services according to IIH service definition and supervision requirements stipulated in Rule 10A NCAC 27G.
0104 and to licensure & certification requirements of the appropriate discipline.
The primary purpose of this position is to provide an intensive team approach that provides direct treatment and restorative interventions as well as case management to minimize the risk of out of home placement.
IIH Team Lead serves as the primary contact and coordinator of services supporting individuals with Mental Health or Substance Abuse Issues in residential, school, workplace and community settings Major Duties and Responsibilities Team Lead must provide direct clinical interventions with each child.
Designate the appropriate team staff such that specialized clinical expertise is applied as clinically indicated for each child Provide and Coordinate assessment and reassessment of the recipient's clinical needs Provide clinical expertise and guidance to the IIH team members in the team interventions with the recipient Provide clinical supervision of all members of the team for the provision of this service.
An individual supervision plan is required for all IIH team members exclusive of the Team Lead Coordinate and oversee initial and ongoing assessment activities Convene the Child and Family Team meetings for person-centered planning Complete the initial development and ongoing revision of the Person-Centered Plan and ensure implementation Consult with identified medical (for example, primary care and psychiatric) and non-medical (for example, DSS, school, DJJDP) providers, engage community and natural supports, and include input in the person- centered planning process Ensure linkage to the most clinically appropriate and effective services including arrangement for psychological and psychiatric evaluations Provide and coordinate behavioral health services and other interventions for the youth or other family members with other licensed professionals and Child and Family Team members.
Provide parenting skills training to aid the family build fundamental skills needed for coping with the youth's disorder Participates in agency first responder on-call system that provider crisis intervention 24/7/365 as indicated in the Individual's crisis plan Ability to evaluate the nature of the crises, intervene and diffuse the crises likelihood of a recurrence Provide or arrange crisis management, intensive case management, individual therapy, substance abuse intervention, skills training, and other rehabilitative supports to prevent the need for out-of-home, more restrictive services Monitor and Document the status of recipient progress and the effectiveness of the strategies and interventions outlined in the Person-Centered Plan Responsible for maintenance and implementation of a schedule of contacts by staff for each individual being served Responsible for oversight of services received and making certain that the appropriate amount of service is being provided in accordance with the service definition Maintain confidentiality and comply with governmental laws and regulations, as well as Company policies and procedures and all other duties and responsibilities as assigned per State and/or Federal Service Definition(s) and/or Clinical Policy 8A.
Contribute to the team by completing any other duties and responsibilities as assigned by SEBHS Director(s), Manager(s), Supervisor(s), and/or other SEBHS Authorized staff.

Agency required in-house trainings IIH Service Definition Training/Clinical Coverage Policy - 3 hours Crisis Response Training - 3 Hours PCP Instructional Element Training - 3 Hours IIH Service Note Documentation Training - 3 Hours Within 90 days of hire:
Introduction Motivational Interviewing (MI) Training by a MINT Trainer (2 days) - 13 hours Introduction to System of Care Training - 11 hours Person-Centered-Thinking Training from a PCT Trainer - 12 Hour IIH Team Leaders shall complete all supervisory level training required by the developer of the designated therapy, practice or model.
If no specific supervisory level training exists for the designated therapy, practice, or model, then all IIH Team Leaders must complete a minimum of 12 hours of clinical supervision training.
A minimum of 24 hours of training (a minimum of 3 days) in one of the designated therapies, practice or models below:
Cogitative Behavioral Therapy Trauma-Focused Therapy Family Therapy Minimum Qualifications IIH Team Member must meet the requirements for Team Lead and have the skills, knowledge, and abilities required by the population and age to be served.
Also, must have a minimum of one (1) year documented experience working with MH/DD/SA population.
IIH service providers focused on substance abuse must include a Certified Clinical Addiction Specialist, or a Certified Substance Abuse Counselor Certification as defined by North Carolina guidelines.
Licensed Team Lead is an individual who holds a license (i.
e.
LCSW, LCSWA, LCMHC, LCMHCA, LMFT, LMFTA, CCS, LCAS, or LCASA) who has at least one-year experience with the knowledge, skills, and abilities required by the population and age to be served.
The Team Lead shall meet the requirements specified for licensed clinician, according to 10A NCAC 27G.

An associate level licensed clinician actively seeking licensure may serve as the Team Lead conditional upon being fully licensed within 30-calendar months from the effective date of hire.
An PhD or MD Level Clinician can be a Team Lead as long it's within the scope of practice.
A Team Lead may also be a Certified Clinical Supervisor (CCS) or Licensed Clinical Addiction Specialist (LCAS or LCAS Training and experience should fully prepare an individual for this work.
There is no informal training and experience that could substitute for these qualifications.
Clinician should provide individual professional liability insurance up to one (1) million dollars.
Additional Requirements Valid Driver's License, Auto Insurance, and reliable transportation Criminal/DMV background check for the respective state or national if lived in another state, NC Healthcare Registry, Office of Inspector General and National Sex Offender.
Available to accept random drug testing.

Maintaining Professional Boundaries and Ethical Conduct All clinicians are required to follow the ethical codes of their specific license, as well as SEBHS Code of Ethical Conduct.

Duty to Report and Duty to Warn Informed Consent Dual relationships and healthy boundaries Conflicts of interest Safety screening Unjustified gains and dynamics in therapeutic relationships Need for referral to other providers Practicing within professional scope and training.
Coordinating Services Often individuals may be involved in other formal treatment services such as case management and medication management, or more informal services such as support groups, 12-step groups, and peer groups.
Clinicians providing outpatient services, with written consent, consult and coordinate with other mental health and medical providers to ensure continuity of care.
Coordinating Recommended Services When an individual has a primary substance abuse diagnosis, the individual should be referred to a licensed substance abuse professional.
Accommodating Individuals Clinicians work to accommodate individuals by providing services at convenient times and locations for individuals.
For example, clinicians will offer services during the day and in the evening.
Also, individuals will consider transportation barriers when scheduling appointments and accommodate, when possible, by providing services in the home (if clinically appropriate).
Informed Consent All individuals receiving services have a right to be informed about the clinician's professional and educational background and training.
Additionally, individuals have a right to be informed about their rights, limits of confidentiality, and the right to another referral upon request.
Each individual is to work collaboratively with the clinician in mutual goal setting in the development of an outpatient treatment plan.
Additionally, each individual has a right to be fully informed of their diagnosis and treatment recommendations Medical Necessity After an initial assessment is completed, the Medical Director will be responsible to review the medical necessity of outpatient treatment services by reviewing and signing the outpatient treatment plan or Person-Centered Plan.
Agency Responsibilities These include attendance at required meetings (includes peer and clinical supervision, case staffing/consultations) and trainings to keep abreast of agency regulations and procedures, increase knowledge and skills in clinical areas, and provide needed agency services, documentation of clinical work with individuals and maintenance of medical records per agency and Medicaid or insurance standards.
Other Position Characteristics Accuracy of work by the individual in this position will have a tremendous impact on treatment outcome for individuals and parents/families.
Exact accuracy with respect to the management of emergencies and the diversion of inappropriate hospitalizations are required.
Effective as well as efficient direction of allocation of fiscal resources is mandatory in supporting and realizing the primary goals and objectives of children's services.
This accuracy requires visual/perceptual, verbal and written communication skills.
Consequences are commensurate with the error, ranging from inconsequential to severe.
Knowledge of SEBHS guidelines and policies, confidentiality guidelines, professional standards for treatment, DSM V, and LME-MCO Program guidelines.
Ongoing supervision meetings with supervisor, staff meetings, and Child and Family Team meetings (if indicated).
Regular contact with SEBHS staff (clinical and administrative) some contact with parents/guardians and extensive contacts with staff from other community agencies, schools, and juvenile justice staff.
Minimal physical effort should be required for the fulfillment of the requirements of this position.
The tasks of this position generally will be performed in a community or office setting.
Visual observation of staff's interactions with individual's and peers is essential for training and ongoing evaluation purposes.
Visual observation is also essential to the individual's treatment and proper allocation of resources.
Intense mental concentration is necessary for the following:
managing an individual through crisis, fulfilling medical record requirements and other essential paperwork, communicating with officials in the community, evaluating potential emergency placements, etc.
All possible safety measures will be met to protect individuals and other staff if the need should arise.
Knowledge, Skills, Abilities This position requires a thorough knowledge of evidenced based therapy techniques; broad knowledge of principles of child development, positive behavioral supports and interventions, and life skills activities; in depth knowledge about community mental health and related health systems; minimal competency with personal computing; and skills in the planning, development and implementation of programming goals and objectives.
Effective communication skills are essential.
Level of Authority Performs daily routines independently in accordance with established and general policies and procedures requiring regular interpretation.
Is responsible for sound judgment, thoroughness and competence, where failure to perform effectively and efficiently could have serious impact on company operation, individual plans and the efficient use of resources.
This position is supervised by the Program Director and/or Clinical Director.
Working Conditions The Licensed Professional may be required to work within the facility, outside the facility, community, school, and/or within recipient home.
Flexible hours required.
The Licensed Professional & team shall provide first responder crisis response on a 24/7/365 basis to recipient receiving IIH service.
Travel using personal vehicle will be necessary and will also require a DMV background check and proof of personal automobile insurance.
Hiring/Termination Executive Management Team and/or assigned personnel may authorize hiring and/or termination of this position.
Employee is required to fulfill all regular job assignments, duties, and obligations of their position until final date of employment.
All company property which includes individual's books, service notes, PCP plans, company purchased supplies, training materials, etc.
must be returned before receiving final paycheck(s).
