5K a year Job Purpose Community Support Teams (CST) Qualified Professional is responsible for providing CST services according to CST service definition and supervision requirements stipulated in Rule 10A NCAC 27G.

The primary purpose of this position is to provide an intensive community-based rehabilitation team services that provides direct treatment and restorative interventions as well as case management Major Duties and Responsibilities (Under the direction of the Team Lead) Supervision must occur according to licensure and certification requirements of the appropriate disciplines Participating in the team weekly case staffing meeting Provides psychoeducation as indicated in the PCP Assisting the Team Lead in monitoring and evaluating the effectiveness of interventions, as evidenced by symptom reduction and progress toward goals identified in the Person-Centered Plan Assisting the Team Lead in consulting with identified providers, engaging community and natural supports, and including their input in the person-centered planning process Assist with crisis interventions 24/7/365 as indicated in the Individual's crisis plan Assist with behavioral and substance use disorder treatment interventions Complete the initial development and ongoing revision of the Person-Centered Plan and ensure implementation Provides clinical oversight and coordination of Associate Professionals and Paraprofessionals in implementing interventions in accordance with the PCP Working closely with other clinical/professional staff to maintain communication and providing feedback, standardizing procedures and expediting PCP implementation Consult with identified medical (for example, primary care and psychiatric) and non-medical (for example, DSS, school, DOJ) providers, engage community and natural supports, and include input in the person- centered planning process Ensure linkage to the most clinically appropriate and effective services including arrangement for psychological and psychiatric evaluations Provide and coordinate behavioral health services and other interventions for the Individual Completes functional needs assessment(s) to determine the scope and anticipated outcomes to the services Assist with the development of relapse prevention and disease management strategies Provides and assist with intensive case management to include Linkage and arrange for services and referrals Monitoring and follow up Assist Individual with housing search including engaging landlords to rent to the Individual and writing reasonable accommodation letters Assist with connecting Individual's to financial and in-kind resources to set up and maintain their household Prevent and mitigate housing crises including being a point of contact for landlord concerns Assist with rehousing the Individual if they are no longer able to stay in their unit due to eviction or risk of eviction Assist in developing daily living skills to stabilize and maintain housing CST maintains an Individual-to-staff ratio of 12:
1 with a team maximum of 48 individuals All members of the CST services team shall be supervised by the Team Lead.
Persons who meet the requirements specified for Qualified Professional and who have the knowledge, skills, and abilities required by the population and age to be served may deliver CST services Maintain confidentiality and comply with governmental laws and regulations, as well as Company policies and procedures and all other duties and responsibilities as assigned per State and/or Federal Service Definition(s) and/or Clinical Policy 8A.
Contribute to the team by completing any other duties and responsibilities as assigned by SEBHS Director(s), Manager(s), Supervisor(s), and/or other SEBHS Authorized staff.

Follow up training and ongoing continuing education required for fidelity to chosen modality (If no requirements are designated by developers of that modality, a minimum of 10 hours of continuing education in components of the selected modality must be completed - 10 Hours (Modalities must be ONE of the following:
Cognitive Behavioral Therapy, Trauma Focused Therapy, and Illness Management and Recovery (SAMHSA Toolkit) Minimum Qualifications CST Team Member must meet the requirements for Qualified Professional and have the skills, knowledge, and abilities required by the population and age to be served.
Also, must have a minimum of one (1) year documented experience working with MH/DD/SA population.
CST service providers focused on substance abuse must include a Certified Clinical Addiction Specialist, or a Certified Substance Abuse Counselor Certification as defined by North Carolina guidelines.

An individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession, except a registered nurse who is licensed to practice in the State of North Carolina by the North Carolina Board of Nursing who also has four years of full-time accumulated experience in mh/dd/sa with the population served; or 2.
A graduate of a college or university with a Master's degree in a human service field and has one year of full-time, pre/post-graduate degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional who has one year of full-time, pre/post-graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; or 3.
A graduate of a college or university with a bachelor's degree in a human service field and has two years of full-time, pre/post-bachelor's degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional who has two years of full-time, pre/post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling; or 4.
A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full-time, pre/post-bachelor's degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional who has four years of full-time, pre/post bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.
Degrees in non-human service field must include at least twenty (25) credit hours in human service coursework.
Certification(s) in Substance Abuse, Family Therapy or/and other mental health certification(s) highly recommended.
Additional Requirements Valid Driver's License, Auto Insurance, and reliable transportation Criminal/DMV background check for the respective state or national if lived in another state, NC Healthcare Registry, Office of Inspector General and National Sex Offender.
Available to accept random drug testing.

Agency Responsibilities These include attendance at required meetings (includes peer and clinical supervision, case staffing/consultations) and trainings to keep abreast of agency regulations and procedures, increase knowledge and skills in clinical areas, and provide needed agency services, documentation of clinical work with individuals and maintenance of medical records per agency and Medicaid or insurance standards.
Knowledge, Skills, Abilities This position requires a thorough knowledge of evidenced based therapy techniques; broad knowledge of principles of adult development, positive behavioral supports and interventions, and life skills activities; in depth knowledge about community mental health and related health systems; minimal competency with personal computing; and skills in the planning, development and implementation of programming goals and objectives.
Effective communication skills are essential.
Level of Authority Performs daily routines independently in accordance with established and general policies and procedures requiring regular interpretation.
Is responsible for sound judgment, thoroughness and competence, where failure to perform effectively and efficiently could have serious impact on company operation, individual plans and the efficient use of resources.
This position is supervised by the Program Director and/or Clinical Director and/or Medical Director.
Working Conditions The Qualified Professional may be required to work within the facility, outside the facility, community, school, and/or within the Individual's home.
Flexible hours required.
The Qualified Professional & team shall provide first responder crisis response on a 24/7/365 basis to the Individual receiving CST service.
Travel using personal vehicle will be necessary and will also require a DMV background check and proof of personal automobile insurance.
Hiring/Termination Executive Management Team and/or assigned personnel may authorize hiring and/or termination of this position.
Employee is required to fulfill all regular job assignments, duties, and obligations of their position until final date of employment.
